Please download " MotoIMODE_v100_dongle.rar" from Archive folder in support, and use this SW to read the codes.

that is the log from motoimode_v100....
I read a lot, try everything but could not unlock the phone.. maybe unsupported version? I dont have the phone in hand... but I did try everything I read using search button.

I see You getting smart card malfunction there.
Maybe you havent activated the imode slot in smart card ? Can You run dongle client and check the counters status ?
