    Check wires and also try to change SD_CLK Frequency.

          This is controller fault (internal one in eMMC). It used to happen exactly like this in some of the HTC models.
          Put it asside, maybe later there will be some way of reflashing it.

              No, i think such phones freeze at some point.
              Or, this can be caused by bad clock, or watchdog. Reason why You can read 0x200 and not any greater chunk is that internal controller shuts down the power.
              Can be due to "some" fault in internal circuitry, or clock fault.
